Collect E-Signatures in Your Forms

Add a signature field to any form. Respondents draw their signature on a canvas — it's stored and displayed inline in your responses.

When you need a signature — consent forms, contracts, agreements, liability waivers — you shouldn't have to use a separate tool. Formsout adds a signature drawing pad directly to your form. Respondents sign with their mouse or finger on mobile, and the signature is stored as a PNG image in your response dashboard.

Signatures are uploaded securely to Supabase Storage and displayed inline when you review responses. Include them alongside other form fields — name, date, checkboxes — for a complete document-style form. All signature images are included in individual response views. The signature field is available on Pro plans and is Pro-gated in the form editor.
