When you need a signature — consent forms, contracts, agreements, liability waivers — you shouldn't have to use a separate tool. Formsout adds a signature drawing pad directly to your form. Respondents sign with their mouse or finger on mobile, and the signature is stored as a PNG image in your response dashboard.

Signatures are uploaded securely to Supabase Storage and displayed inline when you review responses. Include them alongside other form fields — name, date, checkboxes — for a complete document-style form. All signature images are included in individual response views. The signature field is available on Pro plans and is Pro-gated in the form editor.

How it works

Add a signature field
In the form editor, add a new question and select 'Signature' as the type. Name it ('Signature', 'Sign below', etc.).

Respondent signs
The form shows a drawing canvas. Respondents sign with mouse or touchscreen — they can clear and redo as needed.

View in responses
The signature appears inline in your response dashboard — displayed as an image next to all other answers.
